OFFICIAL RULES

1. NO PURCHASE OR CONTRIBUTION IS NECESSARY TO ENTER OR TO WIN.  A PURCHASE WILL NOT IMPROVE YOUR CHANCES OF WINNING.  ALL FEDERAL, STATE, LOCAL AND MUNICIPAL LAWS AND REGULATIONS APPLY.  VOID WHERE PROHIBITED.
2. Eligibility.  Subject to the additional restrictions below, the Dental Care of Mid Florida (DCMF) Shopping Spree Giveaway is open to Florida residents 18 years or older at the time of entry whom are patients of record and have referred one or more patients to DCMF during the entry period.  Employees of Dental Care of Mid Florida and the immediate family members of, and any persons domiciled with, any such employees, are not eligible to enter or to win.  The term immediate family members includes spouses, parents, grandparents, siblings, children and grandchildren.

3.  How To Enter.  The entry period begins at 12AM on 8/29/19 through 12/6/2019.  Dental care of Mid Florida reserves the right to extend the entry period. Starting on or about DECEMBER 9, 2019 through DECEMBER 13, 2019, you will receive an entry to the giveaway for each qualified referral at the following locations of DCMF:

A qualified referral is defined as any current patient of record (the referring patient) who refers another individual that is not a patient of record (the referred patient) and the referred patient then becomes a new patient of record at any of the DCMF locations.

1. Winner Selection. Five (5) Grand Prize Winners will be selected on or about DECEMBER 9, 2019 in Sebring, FL.  You must be present to win.  The winners will be randomly selected from all of the entries.  The drawing for the five (5) winners will take place on or about DECEMBER of 2019. The Winner must provide a valid driver’s license at the time of pickup of the grand prize.  Prizes may not be transferred before pick up by the selected winner.
2. Prizes and Odds.  Grand Prizes: Five (5) Grand Prize Winners will receive a $1000 Visa Gift Card.

Winner is responsible for any sales taxes and any personal income taxes owed according to IRS rules.  A 2019 1099-MISC will be issued to the grand prize winner.  Odds depend on the total number of entries received during the entry period.

1. Winner Notification and Acceptance.  The five (5) winners will be notified by phone or email on or about December 2019.  To claim the prize, the winner must respond back to Dental Care of Mid Florida via phone or email within 7 days to claim the prize or the prize will be subject to forfeit and will be awarded to the next eligible entry. 
   1. Winners may waive their right to receive prizes.  Prizes are non-assignable and non-transferable.  No substitutions allowed by winner. In the event any winner waives their right to receive any of the prizes, DCMF reserves the right to award any waived prizes to the next eligible entry. Participation.  By participating, entrants agree to be bound by these Official Rules and the decisions of Sponsors which shall be final in all aspects.  Sponsor reserves the right to disqualify persons found tampering with or otherwise abusing any aspect of this stated expiration date, Sponsor reserves the right to award prizes based on entries received before the termination date.
2. Construction.  The invalidity or unenforceability of any provision of these rules shall not affect the validity or enforceability of any other provision.  In the event that any such provision is determined to be invalid or otherwise unenforceable, these rules shall be construed in accordance with their terms as if the invalid or unenforceable provision was not contained therein.
3. Sponsors.  The Dental Care of Mid Florida Shopping Spree Giveaway is sponsored by Dental Care of Mid Florida.  The decisions of Sponsor and contest judges regarding the selection of winners, the interpretation of these Official Rules and all other aspects of the contest shall be final and binding in all respects.  Sponsor will not be responsible for typographical printing or inadvertent errors in these Official Rules or in other materials relating to the Contest.  If you have any questions regarding this Contest, call 863-382-9090 during regular business hours.
